094_ Drugs Capable of Remyelination and Cognitive Performance in Psychosis and Affective Disorders
Research Question and Aims
Brain imaging findings suggest a decrease in number and density of oligodendrocytes specifically in hippocampal regions in patients with schizophrenia and other psychotic disorders (PMID: 27460617, 31552495) presumably affecting axon myelination resulting in observable cognitive deficits (PMID: 27065804, 37163675). Treatment strategies targeting myelin and oligodendrocyte dysfunction have been discussed as promising approaches in schizophrenia (PMID: 32425837), with certain antipsychotics assumed to contribute to such effects. Most conclusive data have been obtained for quetiapine fumarate, showing in-vitro oligodendrocyte differentiation (PMID: 27592856) and remyelination (PMID: 24997607). Own preliminary data suggest that quetiapine treatment in major depression may have beneficial effects on cognitive flexibility. We wish to further investigate these effects in the PsyCourse cohort, by hypothesizing that quetiapine treatment with continuous daily dosages of 200 mg/day or more shows beneficial effects on global psychosocial functioning and cognitive performance compared to patients without quetiapine treatment or with low dose quetiapine treatment up to 100 mg/day.
Analytic Plan
We assume the strongest effect for quetiapine treatment of >= 200 mg/day vs. <= 100 mg/day for patients with psychotic disorders, given the prominent role of oligodendrocyte pathology in these patients. To ensure continuous antipsychotic treatment, we wish to select patients meeting medication criteria at two consecutive visits. In addition, we would like to repeat the same analysis also in patients with affective disorders, where we also assume effects albeit at a smaller effect size. Primary outcome is change in cognitive function, with the main focus on Trail-Making-Test B and Digit-Symbol-Test. Secondary outcome is the global assessment of functioning (GAF), with clinical symptom assessments PANSS, IDS-C30, YMRS, and BDI-II as additional exploratory outcomes. Our own preliminary data suggest medium to large effect sizes for certain aspects of cognition, which should be achievable with a minimum number of patients of N = 60 per comparison group.
